Pre-surgical Bariatric Evaluations

 

Want the best chance to keep your weight off after a bariatric surgery? An evaluation with Dr. Kutz can help you understand what your struggles might be and how to effectively manage them.  Many bariatric surgeons require evaluations to clear patients for surgery, but Dr. Kutz provides more than just an approval.  She will work with you to help you understand your struggles with weight loss and help you find a path to health.

Fees: $205 per hour of service (typically between $1,025 and $1,640)

Dementia and Capacity Evaluations

 

As a board certified Geropsychologist, Dr. Masuda offers cognitive assessment for diagnostic clarification and treatment planning purposes. Capacity evaluation is available and may include assessment of financial, medical decision making, or independent living capacities.

Fees: $205 per hour of service (typically between $1,640 and $3,000 to accommodate for testing, report writing and feedback)
